Child's Name: Write the full name of your first child. Date of Birth Enter the birth date of your first child. Child's Name: Write the full name of your second child, if you have one. Date of Birth Enter the birth date of your second child, if you have one. Child's Name: Write the full name of your third child, if you have one. It is in the best interest of the child(ren) to grant me primary physical custody because (explain): Explain why your child is better off living mainly with you. Include things like who cares for them daily, their safety, and their needs. If I am allowed to move with the child(ren), I propose the following visitation schedule: (Describe what visitation schedule you are proposing, and why it will be adequate) Write out the visiting plan you want for the other parent after you move, such as which holidays, summers, or weekends. Explain why this plan gives the children enough time with that parent. If the move is allowed, I will comply with all substitute visitation orders.
