A sworn written statement used to tell the court facts that support your position — signed under penalty of perjury.
What this form does
MC-030 is a general declaration form — used whenever you need to submit a sworn factual statement to the court. You write out the facts you want the judge to know, sign under penalty of perjury, and attach it to your filing. Declarations are how parties tell the court what happened when they are not testifying in person.

Any party or witness with facts relevant to a court matter.
When to file it
File with any motion or response when you need to submit factual statements to the court.
What happens after you file
The judge reads your declaration as part of the court record. It carries the same weight as testimony.
