Declaration of Disclosure
Cover sheet confirming you have completed your financial disclosure — served on the other party, not filed with the court.
What this form does
California requires both parties in a dissolution to exchange complete financial disclosures. FL-140 is the cover sheet confirming that exchange happened. It lists what was disclosed and when. Importantly, FL-140 itself is not filed with the court — it is served on the other party. Only the proof of service is filed.
Who needs to file it
Both parties in a dissolution case.
When to file it
Served on the other party early in the dissolution process. Both parties must complete their disclosure.
What happens after you file
The other party receives your financial disclosure package. You file proof of service with the court.
