Proof of Service of Summons

Confirms that your spouse or domestic partner was personally served with the divorce petition and summons.

What this form does

After the respondent is served with the FL-100 and FL-110, the process server completes FL-115 describing when, where, and how service was accomplished. This is filed with the court as proof that the respondent was properly notified. Without it, the court cannot proceed.

Who needs to file it

Completed by the person who served the papers — not the petitioner.

When to file it

File after service is completed, before requesting any default or proceeding.

What happens after you file

The 30-day response window begins running from the date of service reflected in FL-115.
