Proof of Personal Service
Confirm that someone other than you personally handed court documents to the other party.
What this form does
California requires that the other party be personally served — meaning an adult who is not you hands them the documents in person. After service happens, the person who did the serving fills out FL-330 describing when, where, and how service occurred. This form is filed with the court to prove service happened.
Who needs to file it
Completed by the process server or adult who served the papers — not the person filing the case.
When to file it
File with the court after the other party has been served. Required before the court will proceed.
What happens after you file
Once filed, the court has confirmation that the other party was properly notified and the case can move forward.
