Proof of Personal Service

Confirms that the restrained person was personally served with the restraining order — required before it can be enforced.

What this form does

Before a domestic violence restraining order can be enforced, the restrained person must be personally served with the TRO and hearing notice. DV-200 is the proof that service happened — completed by the person who served the papers. Without it, law enforcement cannot enforce the order.

Who needs to file it

Completed by the person who served the DVRO papers — law enforcement, a process server, or any adult not named in the case.

When to file it

File after the restrained person has been personally served.

What happens after you file

Law enforcement can now enforce the order. The hearing can proceed.
