Description of Abuse
Tell the court what happened in your own words — filed alongside the DV-100 request for a restraining order.
What this form does
The DV-101 gives you space to describe the abuse in detail. The DV-100 has limited room — the DV-101 is an attachment that lets you tell the full story. Courts read this when deciding whether to grant emergency protection. Be specific: dates, locations, what was said, what was done, injuries, witnesses. The more specific you are, the stronger your filing.
Who needs to file it
Filed by the person requesting a domestic violence restraining order when they need more space than the DV-100 provides.
When to file it
File with your DV-100. If the DV-100 does not have enough room for your description, attach the DV-101.
What happens after you file
The judge reads both forms together. The DV-101 gives context that can be decisive in whether the TRO is granted.
