What this form does
The FL-150 is a sworn financial snapshot — your income from every source, your monthly expenses, and your assets. Courts use it to calculate child support and spousal support. It has to be accurate because you sign it under penalty of perjury. If your finances change, you may need to update it.
Who needs to file it
Required in any case involving child support, spousal support, or attorney fees. Both parties typically file one.
When to file it
File with your Request for Order or Response when support is at issue. Update it whenever your financial situation changes significantly.
What happens after you file
The court compares both parties' FL-150s to determine support amounts. Inconsistencies or missing information will be questioned at the hearing.
